These deformations mainly occur on advanced and old hallux valgus. Indeed, due to the loss of support of the big toe, the mechanical stresses are transferred to the lateral rays.
This concerns plantar and back pain located upstream of the 2nd and 3rd toes. At this stage, treatment of hallux valgus is usually not enough and it must be combined with osteotomies of the lateral rays.

The postoperative course is identical to the treatment of isolated hallux valgus, although the foot is initially more edematous. Rehabilitation is fundamental.
At the later stage, the toes become irreducibly clawed, causing conflict with the leather of the shoe. Calluses then appear on the back of the toes.
